Documents required for registration  

1.  Child’s birth certificate or passport
2.  If applicable, Social Security Number
3.  If applicable, proof of guardianship
4.  Proof of address
5.  Immunization records including diptheria, varicella, tetanus, whooping cough, polio, hepititus B and two doses of measles/mumps/rubella (MMR) *
6. Transcript from previous school (if transferring from out of parish)


* Louisiana state law requires students entering any Louisiana school for the first time to show proof of immunization against several diseases.
